Employment at Will I understand that, if hired, my employment is at will for no definite period and may be terminated at any time without prior notice. I recognize and agree that my reliance on any statements by supervisory and/or management personnel contrary to this at will relationship is unreasonable.

Drug-Free Workplace CresCor maintains a drug-free workplace. All applicants for certain positions (based job duties) must undergo a pre-employment drug screening at CresCor's expense. Applicants testing positive for illegal substances will be disqualified from consideration. Upon hire, employees will be expected to abide by the company's drug testing policy.

Physical Examinations All new hires for certain positions (based on job duties) will be required to undergo a company-paid physical examination. All information from this examination will be kept confidential and disclosed only to supervisors, managers, and safety or rescue personnel who have a need to know. No one will be disqualified from employment based on the results of this examination without CresCor first weighing possible accommodations and establishing the legality and business necessity of the physical requirement.

Employment Eligibility To comply with the federal Immigration Reform and Control Act, CresCor requires all new hires to show proof of their eligibility to work in the United States. Failure to produce the required documents will cause CresCor to withdraw its job offer and terminate an individual's employment.

Reference Check Authorization I understand that an investigative report may be made and hereby agree to authorize all persons, schools, companies, consumer reporting agencies and other organizations to supply any accurate information concerning my background. I authorize investigation of all statements contained herein and the references listed to give you any and all information concerning my previous employment and any pertinent information they may have and release all parties from all liability for any damage that may result from furnishing same to you.
